Blepharoplasty – Eyelid Surgery

One of the most common procedures I perform in my Fourways rooms is Eyelid surgery, or Blepharoplasty.

serEyeBlog

This is because, unfortunately, the eye area is one of the first areas to show the signs of aging.

Indeed, many men and women suffer from droopy or hooded eyelids, either due to aging, heredity factors, or both. And the same applies to the wrinkles, puffiness and fat bulges that can develop in the lower eyelids. These conditions can be particularly bothersome, since the eyes are such a critical part of our expressions and our interactions with others. Patients often complain of appearing tired or unhappy despite feeling otherwise.

Eyelid surgery always has a tremendous impact on the overall appearance of the face and gives my patients a younger, more awake and energetic look that boosts their confidence.

Contact my Fourways plastic surgery practice on 011 875 1631 to learn more about the eyelid surgery.

IS BLEPHAROPLASTY THE RIGHT SURGERY FOR YOU?

Eyelid surgery is a wonderful procedure for restoring youthfulness to eyes that are showing severe signs of ageing resulting from gravity, time and genetics. During this procedure, I remove fat pockets and tighten the muscles but I explain the procedure in more details further on in this article.

The best candidates for this plastic surgery procedure are healthy men and women who are in their 30s or older and who have realistic expectations.

Of course, as with all cosmetic procedures, it is especially important that I carefully assess your eyes and the surrounding areas and understand your goals and concerns before recommending eyelid surgery.

During your consultation, I will determine if you truly have excessive eyelid skin, or if your sagging eyelids are mainly caused by a drooping brow. If this is the case, you may actually need a brow lift, either with or without blepharoplasty. Find out more about brow lift on my website

I will also decide whether you require upper eyelid surgery, lower eyelid surgery, or both, and what approach would be best suited to your condition.

Since the eyes and the eye area are such an important part of our appearance, it is extremely important that eyelid surgery be performed only by a board-certified plastic surgeon specializing in this delicate procedure. I, myself am a member of the APRSSA.

WHAT IS THE DIFFERENCE BETWEEN UPPER AND LOWER EYELID SURGERY?

This is probably the number one question my eyelid surgery patients ask me.

→Upper Eyelid Surgery

When performing upper eyelid surgery, my goal is to remove excess skin and occasional fat bulges that are responsible for the tired, grumpy look. I make an incision that runs along the natural fold of the upper eyelid, and extends just beyond the outside corner into the existing creases.

Through this incision, a strip of excess skin is removed and fatty tissue is eliminated or repositioned to create a more refreshed, alert appearance. Because the incision follows the natural crease of the eyelid, it is usually very well-concealed and rarely noticeable.

→Lower Eyelid Surgery

While upper eyelid surgery primarily involves the removal of sagging skin, lower eyelid surgery mainly targets the fatty deposits, which are more predominant under the eye. Removing these deposits in conjunction with the sagging skin leaves a smooth, firm, younger-looking surface above the cheek.

Lower eyelid surgery may be done through the inside of the lower eyelid, using what is called transconjunctival or “scar-less” technique which requires no external incision, and is appropriate only for the removal or redistribution of fat. It is therefore not suitable if you are showing severe wrinkling in that area.

I usually complete upper and lower eyelid surgery within one to two hours, it is usually done as an outpatient procedure. Did you know that some of my patients choose to have blepharoplasty done in conjunction with a facelift?

Here is an article I wrote about Facelift surgery at my practice

RECOVERING FROM EYELID SURGERY

After surgery, you will have eyelid sutures in place. They provide support to the eyelid tissue to ensure that the incision lines heal beautifully, without excessive strain or tension. These sutures are removed 5 days after surgery.

My patients usually report mild levels of pain following blepharoplasty but I will prescribe oral medications to alleviate any discomfort you may feel. You should expect bruising and swelling too, at least for the first week after your surgery. After 10 to 14 days, most of my eyelid surgery patients feel comfortable in public and return to work and social activities.

We will schedule follow up appointments so that I can check up on your healing progress. I will remove your sutures within the first week of surgery.

The eyelid surgery results can last for years but remember to be patient with yourself during the healing process. While it is imperative to be active, eat well and get plenty of rest, it is true that people heal at different paces.

I hope you enjoyed this article? It was great for me to share some key facts about blepharoplasty. Non-Surgical Aesthetics: Botox & Dermal Fillers

As aesthetic treatments are becoming more and more advanced, I am also seeing an increase in their popularity at my practice. An increasing number of patients are noticing a lack of volume and elasticity that cause wrinkles and lines as they age yet they are looking for an alternative to a facelift surgery.

Winter has been giving your skin a hard time and summer is not too far away which means that now is the perfect time to give your skin a healthy glow, a smooth texture and a young, more sculpted appearance!

I hope you enjoy what follows!

BOTOX: THE CLASSIC NON-SURGICAL TREATMENT THAT NEVER FAILS MY PATIENTS

Deciding on undergoing Botox and/or dermal fillers injections is a great idea as they will restore your face’s youthful appearance in a very subtle manner with no recovery time incurred!

My website offers more information about Botox

Botox is a wonderful option if you wish to erase frown lines, crow’s feet, forehead wrinkles or lip wrinkles. When I inject Botox into the muscles responsible for these wrinkles, it instantly relaxes them, improving or eliminating the bothersome lines and wrinkles.

The patients I treat with Botox are 65 years old or younger but something worth knowing is that you should not wait for severe, deep wrinkles to develop before you book your Botox appointment! Getting Botox injections before your skin reaches an advanced state of ageing is great to prevent the formation of severe wrinkles. And remember, Botox is not just for the ladies! The treatment’s affects can last from 4 to 6 months.

Of course, touch ups will be necessary to keep your skin smooth and youthful. Botox usually requires top ups which are in fact full dose treatments:

  • Every 3 to 4 months in the first year
  • Every 6 months in the second year
  • Every 9 months in the third year

A Botox treatment can fit in during your lunch break! You will be able to drive and resume your normal activities afterwards. And since the results are so natural and subtle, no one will know!

DERMAL FILLERS

THE SECRET TO SUBTLE, UNDETECTABLE AND LONG TERM YOUNG LOOKING SKIN!

My patients often ask me what the difference between Botox and dermal fillers is

and it is a fact that these two products are indeed completely different and do not lead to the same results. Simply put, Botox paralyses the muscles causing the wrinkles to appear whereas dermal fillers plump out the wrinkles from the inside out. While Botox can only improve wrinkles caused by movement of facial muscles, dermal fillers are best suited for a wider variety of wrinkles and creases, such as nasolabial folds, smile lines, and lip lines. It can also be used for nose reshaping or cheek enhancement!

During the consultation I will assess the signs of ageing your face is displaying and together we will discuss whether to go for Botox or fillers.

If you would like to know more about dermal fillers, visit my website

I use both Restylane and Teosyal dermal fillers for facial aesthetic treatments at my practice. I love how multi-dimensional they are. In using these brands, I get the assurance that I am working with the safest and most efficient products on the dermal fillers market. My goal is to offer predictable and satisfactory results at all times.
During a dermal filler treatment I will inject the hyaluronic acid (which is naturally occurring in the body and a very clear gel) into the skin in tiny drops with an extremely fine needle.

The product also brings water to the surface of skin to keep it looking fresh and supple. I ensure that nothing is over-done such that the results are subtle and natural-looking.

Just like Botox injections, filler injections are often called “lunchtime treatments” because you can be in an out of my rooms within a half-hour!

Restylane and Teosyal work beautifully on those areas that tend to give away your age such as:

    1. Neck
    2. Décolletage
    3. Backs of your hands.

Just as a quick final note, I offer liquid facelifts at my practice. Liquid facelifts beautifully combine Botox and fillers to sculpt, fill up and rejuvenate the face in all the right places!

Mini- & Full- Facelifts

After liposuction, breast augmentation, blepharoplasty (eyelid surgery), abdominoplasty (tummy tuck), breast reduction and rhinoplasty, facelifts are the sixth most requested cosmetic surgery procedure, according to the American Society of Plastic Surgeons (ASPS).

This is echoed in my practice. I often combine Facelift surgery with other procedures.

If you would like to find out more about facelift surgery at my practice, phone my rooms on 011 875 1630 or have a look at my website and remember to keep an eye on my blog where I upload new articles regularly  – any easy way to stay up-to-date is to subscribe.

Who Would Benefit from a Mini / Full Facelift Procedure?

The best candidate for a facelift is a physically healthy man or woman who is realistic about their expectations, and interested in improving sagging facial skin, jowls, and loose neck skin.  It is to be noted that both men and women are equally interested in Facelift surgery.

The full Facelift is well suited to you, if you have:

  • Excess or sagging facial skin
  • Deep facial creases
  • Facial fat pockets drooping
  • Or if you look tired, sad and angry even when well-rested

Most patients undergoing full facelifts are over the age of fifty.  If you are in your 50s or 60s and have a traditional (full) facelift, your face will age slower, from the point of where it has been reset, and you’ll look at least 10 years younger.

There is obviously more recovery needed from undergoing a full facelift, than when compared with a mini-facelift.
The mini facelift is appropriate for younger people who are starting to show early signs ageing like droopy jowls, or people who have already had a lift, but once again are showing some signs of sagging.

Mini facelifts generally erase three to eight years from your face. After a Mini facelift you will continue to age naturally and the procedure can be repeated in the future.

You may also opt for a full face lift in the future. However, during your consultation I will determine which facelift is best for you.

Irrespective of your age, you can expect a facelift to improve your appearance. You should appear younger, with a well-rested, healthy look. People should ask you if you have been on sabbatical…

Combining Facelift surgery with Blepharoplasty surgery will also enliven your eyes. I do offer Blepharoplasty surgery at my practice. Take a look at this blog post for more info

The truth is, the younger you are the longer you can expect the facelift results to last.
If you are older, you will experience a more dramatic improvement, however, there is also a greater possibility of tissue rebounding. This is due to loss of skin elasticity. We can try to improve skin elasticity with dermal filler injections of fat fills, but we can talk about this during your appointment.

Males Seeking Mini / Full Facelifts:

Technically speaking, a male facelift is not all that different from a female facelift. There are, however, some gender differences that affect the position of the incisions and facelift recovery

Facelift key Differences: Women vs. Men

  • Men have thicker skin
  • Bleeding risk is higher in men
  • Men have more hair follicles on their face
  • Incisions may be harder to mask in men
  • Pulling skin back may move hair follicles; affecting shaving

Personalising each Facelift:

The incisions in front of and behind the ear from the facelift are usually not noticeable. For each individual case, I will make some adjustments to ensure patient satisfaction.

How  are Surgical Facelifts Performed?

Having a surgical facelift will tighten and pull together the sagging skin, wrinkled skin, and loose muscles to leave you looking younger and refreshed. This procedure will reshape the aging face by repositioning the underlying tissue and tightening the skin.

Techniques of today have made Facelift surgery more predictable, safer. And give a far more natural look.I can perform a full face and neck lift, a neck lift alone, or a modification of either procedure depending on your specific needs.Techniques today allow for individual surgical approaches and I will be able to determine exactly what kind of surgery is best suited for your specific characteristics.

The facelift results of today are far superior to those of a few decades ago. In the past, a facelift mainly involved pulling the skin tighter, which created an unnatural, tight appearance.  We want to avoid that “tight windblown” look now by repositioning fat pads, adding fat  and tightening ligaments in the face for a more natural and long lasting result.

Redefining Facelifts:

I like to focus on restoring the underlying muscle and tissues to their more youthful position, for results that are natural-looking, beautiful, and long lasting.

The modern facelift can:

  • Re-create a clean, defined jaw line
  • Restore the more youthful contours of the cheeks
  • Smooth and firm wrinkled, sagging skin in the lower face and neck

Will there be Visible Scarring?  & What is the Recovery Like?

During a traditional facelift, an incision is made in front of the ear extending up into the hair or hairline. The incision stretches downward in front of the ear, comes around the ear lobe and then upward behind the ear. It then ends in the hair or hairline behind the ear. This incision is placed in the hairline to hide the scars and make your surgery almost invisible.

Following facelift surgery, you would spend the night in the hospital. Pain is uncommon, but I will prescribe medication to ensure your comfort. However, my years of experience and research have helped me develop innovative techniques to minimize tissue trauma and allow for a shorter, more comfortable procedure and recovery.

Generally speaking, for most of my facelift surgery patients the recovery period is within 10 -14 days. For neck lifts, and short scar mini-lifts, the recovery can be even faster.

I hope you enjoyed these daily facts as much as I enjoy sharing them with you! Remember that I would really like to hear your thoughts about any kind of procedure or treatments so do not hesitate and post your questions or comments – any easy way is to connect via Facebook or visit my website.

The Best of Rhinoplasties

Rhinoplasty, also known as nose surgery or “nose job,” is the process of using cosmetic surgery to reshape the nose. It is a surgical procedure that can alter the size and/or shape of the nose, as well as relieve some breathing problems.

It is a misconception that all Rhinoplasty Surgeries are for cosmetic purposes; many people require medical intervention to assist in breathing and/or reconstruction from trauma.

serrNose

Read more about what’s available at my Fourways practice here www.drserrurier.co.za and contact me via phone or email for more details or to schedule a consultation. My office can be reached at 011 875 1630.

WHO IS RHINOPLASTY FOR?

Rhinoplasty can be purely cosmetic or can be used to correct physical problems (such as breathing problems).  In most cases we, plastic surgeons, treat the aesthetic aspect and work with ENT surgeons to correct physical nasal issues.

Amongst the aesthetic problems that I can address:

  • Nose width
  • Nose length
  • Profile issues
  • Large or uneven nostrils
  • Crooked noses.

During your first consultation with me, you will be asked to provide a detailed description of how you wish to improve the appearance of your nose and what you are hoping I can do for you.

There will be obvious limitations but being open and honest is part of obtaining your desired result.

I can change the appearance of your nose in amazing ways, but you need to know that I will still be limited by the unique features of your nose, it’s internal and external structure.

Common requests from my patients include:

  • Making a nose smaller,
  • Reducing the bridge of the nose,
  • Narrowing the nose,
  • Making changes to the nasal tip,
  • Lifting a droopy nose
  • Revising a previous rhinoplasty

If you do not like the way your nose looks and are willing to undergo a surgical procedure, and if you are in good physical and mental health, then you are eligible for a rhinoplasty consultation.

Most rhinoplasty surgeons prefer to wait until an individual has completed his or her growth spurt. This means age 15 or 16 for girls, and a little later for boys. That said, I will hardly ever operate on such a young patient.

This is a big surgical procedure with long term results, it is important that the decision be made as an adult taking into account limitations in future surgery.

WHAT HAPPENS DURING RHINOPLASTY SURGERY?

During the rhinoplasty surgery incisions are made inside of the nose including a small incision made on the underside of the nose between the nostrils. This allows me to perform an “open rhinoplasty”.

→What about the scars? The scars are well hidden and will almost be invisible once healing is complete.

I often decide upon the open rhinoplasty approach because it allows me greater visibility of the internal nose structures. Swelling and bruising however lasts longer with open rhinoplasty. I will discuss his approach with you based on your specific case.
Closed rhinoplasty is a surgical approach to alter the shape of the nose where minimal alteration is required. Often recovery is quicker and swelling is less using this approach but I only prefer this method for common rhinoplasty complaints:

  • Remove a bump from the bridge of the nose
  • Elevate the tip of the nose

WHAT YOU NEED TO KNOW ABOUT THE RECOVERY

After Rhinoplasty surgery you will notice significant swelling and bruising. I always warn my patients that swelling and bruising can last between 1 and 6 weeks for closed rhinoplasty, and longer for open rhinoplasty.

A splint will be applied to the dorsum of your nose that will have to remain in place for at least a week. Once the splint is removed however, the change to your nose will be immediately visible.

80% of the swelling, after Rhinoplasty, will lessen within 6 weeks. The remaining 20% may require up to a year but this remaining swelling is barely noticeable and is mostly internal; during this healing period the nose will become more refined.

Most patients, post Rhinoplasty surgery, are able to return to work any time between 7 to 10 days after their Rhinoplasty. Strenuous activity is limited up to six weeks, so too is exposure to the sun and extreme heat. This is more to allow additional time for healing and preserve the cosmetic result.

In some cases, after the Rhinoplasty, the tip of the nose can be numb for a few weeks. This is completely normal and will resolve within a few weeks without treatment.

Expect to see good results after about four weeks, but the final results are commonly seen after about 9 to 12 months. I always tell my patients that sometimes, minor changes and improvements occur even months after rhinoplasty.

Rhinoplasty results are permanent but, patients are to remember ageing may affect the results. This is of course natural ageing and has nothing to do with the overall Rhinoplasty result.

Looking Good after Weight Loss

LOOKING GOOD AFTER WEIGHT LOSS: POST-BARIATRIC SURGERY AT MY PRACTICE

ser5

Losing weight through bariatric surgery is an incredible accomplishment that requires steadfast commitment. In addition to all the health benefits of being lighter and leaner, it’s nice to walk around with a smaller and lighter physique.

The only downfall, for some, is bothersome excess skin that droops and sags off the body after surgery, such that you feel better but look worse. Indeed, patients who have undergone bariatric surgery and the subsequent significant weight loss have numerous but similar areas of concern. These areas usually include:

  • Breast ptosis, or droopiness, as well as a loss of breast volume
  • Sagging of the facial skin
  • Excess abdominal skin and a laxity of the muscles of the abdominal wall
  • Hanging skin under the arm
  • Wrinkling and excessive bagginess of the thighs

I offer several types of cosmetic procedures to treat this problem and this is what I will be talking about this month!

WHO IS A GOOD CANDIDATE FOR POST-BARIATRIC SURGERY?

The best candidates for post-bariatric body contouring are people who have lost a large amount of weight, typically 55 kilograms or more, through bariatric surgery or another approach to weight loss.

Post-bariatric surgery is one of the most complex and invasive procedures your plastic surgeon can perform, so patients need to be healthy enough to withstand the rigors of major surgery and an extended recovery period.

Additionally, post-bariatric surgery is not primarily a weight loss procedure. Patients do typically lose some weight as skin, tissue, and minor fat deposits are removed. However, good candidates have already lost most of their excess body fat with the exception of a few localized deposits.

It’s strongly recommended that post-bariatric patients wait about a year after weight loss surgery to have cosmetic surgery. This gives the body enough time to lose the excess weight and stabilize.

Post-bariatric body contouring candidates should be in good physical and psychological health and have reasonable expectations. These are contouring procedures — not meant to take off extra pounds, but rather to help mold the skin so that it “fits” the new curves more tightly.

POST-BARIATRIC SURGERY PROCEDURES I OFFER

→Body lift is the most comprehensive procedure for post-bariatric surgery patients. Body lifts improves the appearance of the abdomen, thighs and buttocks. Body lifts can target the upper and/or lower body (when combined together, the procedure is referred to as a “total body lift”).

The lower body lift incision extends around the belt area, through which excess skin above the buttocks and outer thighs is removed and the remaining skin pulled tighter.

The upper body lift involves a bra-line excision and the removal of excess fat and skin from the back and waist. Tummy tucks may be incorporated in a patient’s treatment plan to tighten the abdominal muscles and remove excess skin and tissue.

→Liposuction – Liposuction is frequently employed in conjunction with various skin removal procedures to target localized deposits of residual fat that have not responded to diet and exercise.

In most cases, I can perform the treatment through the incisions required for other procedures.

→Tummy Tuck – The abdomen is usually one of the main areas where excess fat collects on the body. After weight loss, considerable excess skin may remain here.

A tummy tuck trims away this excess skin along with any residual fat deposits. For patients with more far-reaching needs, extended tummy tuck is also available.

→Arm lift (brachioplasty) targets the upper arms which can often look flabby even after massive weight loss. The flab is actually extra skin that hangs from the upper arms and is often described as “bat wings.”

The arm lift incisions are usually made in the armpit and on the inside of the arm from the elbow extending to the armpit, and hidden in the natural shadow of the bicep muscle or on the upper arm. Excess skin is removed and the remaining skin is pulled tight and sutured together.

→Thigh lift. After weight loss, hanging skin is often left on the inner and outer thighs. This skin may rub together, causing irritation or limiting a person’s choice of clothing. With thigh lift surgery, I can remove excess fat and skin using incisions concealed discreetly on the upper and inner thighs.

The incision is made where the thigh meets the groin area, and if necessary, an additional incision is made extending from the groin to the knee.

→Thighplasty is helpful to a number of massive weight loss patients, as the thigh deflates with weight loss, creating excess skin in the horizontal and vertical dimensions. In order to remove this excess and re-contour the thigh into a smaller and shapelier appearance, a vertical excision of skin and soft tissue along the medial thigh is required in addition to the standard groin crease incision. This method allows the skin to be tightened circumferentially around the thigh.

→Breast lift repositions saggy breasts higher on the chest. An incision is made, usually around the areola, extending down the breast to the breast crease and horizontally along the inframammary fold.

Excess skin is excised and the remaining skin is pulled taut around the breast mound and sutured into place. The nipple may be repositioned higher on the chest while it remains attached to the underlying blood vessels.

For patients who have lost more breast volume than they wanted, breast augmentation is also an option.

The buttocks are another common problem area after significant weight loss and buttock enhancement will improve the appearance of a flat or saggy behind. Excess skin is removed and one of two options may be employed. First, implants may be placed to augment the butt and make it look perkier.

The other option is a Brazilian butt lift, in which the area around the buttocks is liposuctioned to make them look smaller, and the liposuctioned fat is purified and re-injected into the upper buttocks, giving it a lifted effect.

Combining several procedures in a single surgery

All of the post-bariatric concerns I’ve spoken about cannot be addressed simultaneously during one operation. However, two procedures usually can be safely combined. During your consultation, I will assess your anatomy as well as your own priorities to help you develop a comprehensive plan for your body contouring.

For instance, it is not uncommon for me to combine a tummy tuck with a breast lift (with or without the addition of breast implants), or a thigh lift with an arm lift, as well as other possible combinations of procedures.

Combining the procedures consolidates recovery into a more manageable timeframe. Most patients and plastic surgeons also agree that this approach produces much better, more natural-looking cosmetic results.

How long is the recovery-period after body contouring surgeries?

Typically, body contouring procedures require at least three months for the overall cosmetic results to be fully apparent. This allows enough time for all residual swelling to resolve and for soft issue to soften. Scars usually require 12 months to go through the remodelling process, and, because body contouring procedures require significant incisions, they often are apparent during this time period.

Because post-bariatric surgery is such a highly involved procedure, you should be prepared for an extended period of rest and recovery. Recovery time varies widely depending on the extent of the procedure, but in general you should be prepared to take multiple weeks away from work, and often full recovery may take several months.

How long do the results of body contouring last?

The major factor that affects the longevity of results in post-bariatric body contouring is weight fluctuations. This is the main reason that these procedures are delayed until a patient’s weight is stable for a minimum of 12 months. If patients are able to maintain their weight, long-lasting results can be achieved.

The main issues in this patient population are related to scar migration and soft tissue laxity leading to contour deformities. These are the result of the altered skin/soft tissue characteristics related to the previous obesity

If you are interested in any of the procedures mentioned whether in the context of post-bariatric surgery or not, you must feel free to phone my Fourways rooms on 011 875 1630 and book an appointment!

YOUR RECONSTRUCTION OPTIONS

Post mastectomy most women opt for breast reconstruction. There are a variety of reconstructive options but you may not be a candidate for all types.

Remember: early detection saves lives!

Find out more about the treatments available to you at Milpark via their Breast Cancer website

When to have your breast reconstruction performed is a decision made in conjunction with the other doctors treating your breast cancer, as the need for chemotherapy or radiation will influence when reconstructive surgery can safely take place.

Breast reconstruction is either performed during the same time as a lumpectomy or mastectomy, or at a later date.

If Breast Reconstruction occurs at the same time it is referred to as immediate, and if it occurs at a later date it is referred to as delayed.

Immediate breast reconstruction offers psychological benefits by avoiding a time lapse in addressing any cosmetic change, and also minimizes the number of surgeries for the patient.

Delayed reconstruction is sometimes performed because the patient chooses to wait, or in some cases the patient was not aware at the time that reconstructive options existed.  Other patients are unhappy with the results of their reconstruction and now seek to undergo an alternative surgical method, such as replacing implants with their body’s own natural tissue.

There are a number of breast reconstruction procedures that can help rebuild the breasts. Procedures are tailored to the individual patient, and the type of procedure you have will depend on cancer considerations, your ultimate goals, and your unique body type.

But do not get overwhelmed. We will discuss this in more details during your consultation.

When it comes to breast cancer and breast reconstruction, it’s vital to be surrounded and helped by people who know exactly how to guide you and who can make the process run smoother for you.

The Breast Care unit at Milpark will be there for you every step of the way and will bring you invaluable support in every aspect.

Breast Reconstruction Techniques

Tissue Expansion is one of the breast reconstruction techniques that I offer.

This type of breast reconstruction involves the insertion of a silicone balloon under the skin to expand the tissue in the area. Periodic injections of saline solution cause the skin to stretch and form a pocket for a breast implant. The expander is removed after the growth process is complete and an implant is placed. The nipple and areola can be reconstructed later if the patient desires.

Breast Reconstruction with Implants

Some patients have enough skin in the breast area to place an implant for breast reconstruction without the need of a tissue expander. The implant can be placed under the skin and the nipple and areola reconstructed at a later date.
Did you know that women who need post mastectomy chemotherapy are still candidates for implants?

You will need to discuss all your reconstruction options with me. Your breast reconstruction approach and recovery will depend largely on:

    • Your cancer
    • The need for post-mastectomy radiation
    • Long term risk
    • Skin and tissue quality
    • Desired aesthetic result

Whether you are considering breast reconstruction with implants during or after mastectomy surgery, you will want to discuss with me whether the use of a tissue expander will be necessary.

In addition, you can discuss your options for further reconstruction including re-creation of the nipple and areola.

Have you had breast reconstruction? I would love if you blogged your story – please send us your link. It would be a great help to others.

Flap Surgery

Flap surgery is a method of breast reconstruction that takes living tissue carrying its own blood supply from a donor area of the body to the breast area.

There are a variety of techniques with flap surgery including:

  • TRAM Flap Reconstruction (skin, muscle, and fat taken from the lower abdomen and transferred to the breast area)
  • DIEP Flap Reconstruction (skin, fat, and blood vessels are removed from the abdomen to reconstruct the breast)
  • Latissimus Flap Reconstruction (skin, muscle, and fat taken from the outer back and rotated forward to form the breast mound, followed by implant placement). Nipple and areolar reconstruction can also be performed at a later time.

BETTER SAFE THAN SORRY

The truth is, every woman is bound to experience a breast complaint at some stage of her life and breast concerns can occur at any stage of a woman’s life but most importantly, did you know that 9 out of 10 breast problems are benign and are in no way related to cancer?!

While most women are aware of breast cancer, many forget to take the steps to have a plan to detect the disease in its early stages and to encourage others to do the same.

We’ve come a long way but a lot of progress still needs to be made. Make sure to spread the awareness and share these posts with those around you! When it comes to breast cancer and breast reconstruction, I believe it is crucial to remain aware of all the ways that you can stay proactive and also of all the options that are available to women nowadays.

On the NetCare Breast Cancer page (at Milpark Hospital) you get to read about inspirational cancer survivors’ stories but also about everything related to breast health such as prevention and self-exam.

Do you know what self-examination for breast cancer consists of?

Self-Examinations

By performing self-examinations regularly, you get to know how your breasts normally look and feel which allows you to more readily detect any signs or symptoms if a change occurs, such as:

  • Development of a lump or swelling
  • Skin irritation
  • Dimpling
  • Nipple pain or retraction (turning inward)
  • Redness or scaliness of the nipple or breast skin
  • A discharge other than breast milk

Should you notice any changes you should see your health care provider as soon as possible for evaluation. If you wish to undergo further testing / screening call my Fourways rooms on 011 875 1630

BREAST RECONSTRUCTION AND THE NETCARE BREAST CENTRE AT MILPARK HOSPITAL

This month, I will talk about Breast Reconstruction. Receiving a diagnosis of breast cancer can be devastating, and the possibility of needing a lumpectomy or mastectomy can be incredibly overwhelming. Fortunately however, multiple options for breast reconstruction are available to restore breasts and help you feel stronger than ever.

I believe that breast reconstruction hold tremendous psychological and physiological benefits and feel blessed to be able to offer advanced surgical techniques to my patients.

As a plastic surgeon, I perform lots of breast surgeries such a breast reduction, breast augmentation or breast lift at my Fourways practice.

I also perform Breast Reconstruction surgery at the Milpark hospital.

Did you know that NetCare Breast Centre at Milpark Hospital has an excellent Breast Cancer unit? Breast Care is packed full of informative resources and advice. This makes me even more proud to work in association with NetCare!

I have written many blog posts on breast cancer and breast reconstruction in the past – as raising Breast Cancer Awareness to me is important. You can view them right here

BREASTS PROBLEMS

Problems:

  • You have saggy breasts
  • You desire more cleavage
  • There is a significant difference between one breast and the other
  • You are self-conscious about breasts
  • You feel that larger more perky breasts would make you feel more attractive and improve your self-confidence
  • You have a hard time finding clothes that fit because of discrepancy between your breasts and your buttocks

Solution: Breast Lift!
Based on your answers to the last post, you may or may not also require a breast augmentation at the same time.

After breast lift surgery, you will notice that your breasts are firmer and higher. This will help to improve your confidence and self-esteem. Many patients are happy with the results and report that they feel younger looking and more ‘feminine’.

My Breast Lift page is full of informative resources

Why would I need to combine Breast Lift surgery with Breast augmentation?

Breast lift does not increase the size of your breasts although they may appear larger due to the fact that they are higher and firmer looking. The exception to this is if you have combined this procedure with breast augmentation surgery to enhance volume.

Problems:

  • Your breasts are causing you severe psychological distress, are affecting your physical health or your ability to do exercise.
  • You are experiencing backache, neck and shoulder ache.
  • Your clothes don’t fit properly
  • You do not like the way your breasts affect your overall appearance: large breasts can make you look unbalanced and have a big impact upon your appearance in general. Your breasts are out of proportion with the rest of your body. You feel that you are ‘top heavy’ and this can affect your self-confidence.
  • You do not like being subject to inappropriate comments or attentions from the opposite sex.

Solution: Breast Reduction!

The main benefits are smaller, lighter and firmer breasts. They will be in proportion to the rest of your body. This also means no more physical discomfort such as backache or neck ache. The areola (the brown circular area round the nipple) can also be made smaller and better proportioned as well.

Breast reduction results in one of the most dramatic changes to physical appearance.

  • Your clothes will fit better;
  • You will no longer have physical discomfort
  • Or have to endure unwanted comments about the size of your breasts. This is usually a particular issue for teenage girls.

Problems:

  • You consider your breasts to be too small and would like to increase their size.
  • You feel less than feminine as a result of having small breasts.
  • Your breasts lost firmness, shape and volume after pregnancy
  • Your one breast is smaller/bigger than the other
  • You feel bigger, firmer breasts will give you a more youthful appearance

Solution: Breast Augmentation!

This highly requested surgery will have you feeling happier with your appearance, and will improve self-confidence and well-being. Breast Augmentation is the most requested surgery in my practice and in most cases women are looking for increased size with upper pole fullness.

Visit my breast augmentation page for more info!

The decision to undergo breast augmentation is a highly personal one and all that matters are how you feel about the results. If the results match your expectations then it has been a success.

It can give you a younger looking appearance but it is not a ‘fountain of youth’! As with any form of cosmetic surgery you need to have realistic expectations about what it can do.

STUBBORN FAT PROBLEMS

Problems:

  • You are bothered by excess fat deposits – located anywhere on your body – that don’t respond to diet or exercise.
  • You feel your body is disproportionate (but not necessarily overweight)

Solution: Liposuction and Liposculpture!

The results after liposuction surgery are instantly noticeable. You will have a firmer and more contoured appearance, which will improve your sense of well-being. The effects of liposuction are permanent, which means if you do increase your body fat, then it will NOT return to those areas you had treated.

Liposuction in more detail? Head over to my website!

Liposuction is especially beneficial to those patients who have undergone weight loss surgery. It can remove those last remaining pockets of fat. This will dramatically improve your appearance.

Liposuction can also be combined with other procedures such as abdominoplasty.

However I need to insist on the fact that Liposuction is not a solution to the problem of obesity or weight loss in general.  Also, it is not an effective treatment for cellulite.

Problems:

  • You have recently had a baby and wish to tighten loose skin that has stretched as a result of your pregnancy.
  • You have increased amounts of skin laxity or looseness from the belly button down to the pubic region
  • You have excess skin and fat from the middle and lower abdomen and are looking to tighten the muscles of the abdominal wall to flatten your abdomen

Solution: Tummy Tuck!

During this surgery, I remove excess skin and fat from the waist, which cannot be shifted by diet and exercise alone.

There are three types of abdominoplasty:

  • Complete abdominoplasty
  • Partial or ‘mini’ abdominoplasty
  • Extended abdominoplasty

But don’t worry; together we will discuss which one is best for your needs and characteristics.

Tummy tuck results are impressive: You will have a firmer, toned and flatter abdomen. If you had some lower abdominal bulge as a result of pregnancy then this will also be corrected. This also includes stretch marks! Your waist will appear slimmer and this tighter, youthful abdomen will boost your confidence and self-esteem.

Problems:

  • You do not like the way their nose looks and have been troubled by one or more of its features?
  • You are concerned about the following?
  • Damage to your nose caused by an accident or injury
  • Congenital defect
  • A nose that is too small or too large
  • Excessively flared nostrils
  • An enlarged, drooping or thickened nasal tip
  • Crooked or off-centre nose

Solution: Rhinoplasty!

Rhinoplasty is performed for a variety of reasons which can include increasing or reducing the size of the nose; changing the nasal tip or the bridge, removing any unsightly humps or bumps and making the nose straighter or narrower.

The nasal tip itself can be rotated upwards or downwards, projected forwards or backwards or enlarged or decreased in size. The cartilage, bones and sometimes the soft tissues of the nose are reshaped.

After your rhinoplasty, your nose will be more natural looking and will complement your appearance. Whether larger, smaller or reshaped, you will look and feel better.

The vast majority of my patients are happy with the results of rhinoplasty.

BREAST LIFT

With age, pregnancy and breast feeding and fluctuation in weight, the ligaments and skin envelope which maintain the youthful shape of the breasts, stretch and elongate causing an unpleasant drooping of the breast on the chest wall.

This phenomenon is called ptosis and this is what the Breast Lift will correct.

How does breast lift surgery correct ptosis?
In order to reverse this drooping, I tighten the breast skin and reshape the breast tissue, which in turn will provide a more youthful shape with an improved nipple position. Also, did you know that I could reduce the diameter of your areola if you wish?

Breast Lift and Augmentation is a very common combination procedure!
If fullness and volume are important to you, an implant can be inserted during the breast lift to enhance size or firmness. This is a combination procedure called a Mastopexy Augmentation or Breast lift with additional implants.

Post-operative treatments too, can help reduce the visible scars quickly and effectively. I will discuss your initial reasons for wanting a breast lift and all the options available to you, and thereafter, will take care to ensure that you are satisfied with your new breast shape and scar treatment.

BROW LIFT SURGERY

Over time, frequent facial movements related to facial expressions can lead to a variety of lines and wrinkles in the skin. This is particularly true in the brow area where frown lines and forehead furrows may develop above the nose and across the middle of the forehead.

The brow lift, also known as a forehead lift, is a surgical procedure, which can eliminate lines and furrows in the brow and forehead area. At the same time, the eyebrows may be restored to a more elevated and youthful position and excess skin of the upper eyelids can be eliminated.

The result after a brow lift is a smoother forehead, lifted eyebrows, and an overall more youthful and refreshed appearance to the brow area.

Brow Lift surgery can improve a tired, angry or sad appearance that is often associated with an aged forehead. Specifically, the brow lift can correct the following:

  • Frown lines
  • Forehead furrows
  • Forehead creases
  • Drooping eyebrows
  • Hooding over the eyes

The best candidates for a brow lift are looking to improve aging in the brow and forehead area, are in generally good health, and have realistic expectations.

Other complimentary treatments including Botox®, Fillers and Restylane® Vital. These non-surgical options are available as non-permanent solutions to the position of your brow & forehead lines.

FACELIFT

At my practice, Facelifts are nearly as popular as Breast Augmentations, and I often combine Facelift surgery with other procedures.

Can’t I have Botox and fillers instead of a Facelift?
Although signs of aging eventually appear in all areas of the face, for many people the mid to lower face and neck tend to fare the worst. As skin and tissue lose elasticity, gravity begins to pull them downward.

This is the single most common reason for Facelift Surgery. These areas unfortunately cannot be treated appropriately with Botox and Fillers.

Having a surgical facelift will tighten and reposition sagging skin, wrinkled skin, and loose muscles to leave you looking younger and refreshed. The perfect procedure to boost your confidence in the long term and to start the year feeling fantastic!

The best candidate for a facelift is a physically healthy man or woman who is realistic about their expectations, and interested in improving sagging facial skin, jowls, and loose neck skin.

BUTTOCKS  AND THIGH LIFTS

Now onto the lower body lifts! The lower body lift is performed for patients that desire improvement of the thighs and buttocks, and it is more common in patients after excessive weight loss.

Unwanted excess tissue of the inner and outer thighs can be treated by direct excision of loose skin and subcutaneous fat to improve the contour.

Lateral thigh lift and buttock lift are types of body contouring that remove excess skin and fatty tissue from the outer thigh and buttock regions. Artistic and anatomical removal of this tissue can result in an improved appearance and silhouette.

I always like to emphasize that these procedures are not a cure for obesity, and is most appropriate for persons with localized excess skin and fat bulges that are inherited, or remain despite weight reduction and exercise. And unfortunately thigh and buttock lifts are not a cure for cellulite or stretch marks.

ARM LIFT

Arm Lift will be my last topic of the month. Arm Lift is a technique to remove excess hanging skin from the upper arms usually after substantial weight loss.

I perform arm lift surgery at my practice to remove the excess skin from the upper arms and reduce the circumference of the upper arms.

There are multiple ways to sculpt the arm, including Liposuction, Minimal Access Brachioplasty (axillary scar and liposuction) and Traditional Brachioplasty.

The best candidates for Arm Lift are people with sagging skin of the upper arms. You should be physically healthy, psychologically stable, and realistic in your expectations. Arm Lift will improve your arm contours and remove “Bingo Arms”.
